Health Homes is a free, voluntary care coordination model that promotes and facilitates communication between all caregivers involved in a patient’s care to make sure patient’s get the care and services they need. Patients of Health Homes Program work with personal care coordinators to obtain the tools then need to take back their health.
At Essen Health Care, our Health Homes Program constantly innovate to help address the impact of social determinants of health to our patient’s overall health and well-being.

New York Research Education and Community Health (NYREACH) is a nonprofit organization that focuses on identifying health disparities and provides solutions that will ultimately improve community health. We collaborate with other healthcare and community organizations to provide research, educational opportunities, workforce development initiatives and resources for addressing a variety of health issues.
Founded in January 2015, NYREACH aims to provide a platform for healthcare organizations to conduct community-based health research studies. These studies are used to highlight critical health challenges and implement new community programs for preventing disease and improving health in the greater New York City metropolitan area.
Nursing home division consists of over 100 providers who provide medical care to nursing home residents across new York City boroughs, Long Island and Westchester. Our vision is to create an intergrated system of care to deliver high quality accessible and cost effective medical services for nursing home residents.

In addition to growing its breadth of specialist services and number of locations, over the years Essen has expanded to include hospitalists and physician services in nursing homes to provide a complete spectrum of care delivery in a clinical setting.
Atlas IPA is part of practice management of Essen Health Care. It leads payors negotiations, and support the practice with credentialing and IT solutions needed for the optimal care of patients. It also supports providers within and outside Essen with compliance regulations, administrative contracts and other overhead issues. The IPA aligns providers with a variety of goals including improving quality of care and reducing cost.
